Suggest Treatment For Scrotal Dermatitis

Question: hi im suffering from scrotal dermatitis , there is acute itching especially staring from evening hrs till I sleep, also same itching happens on butts and anal area at times , this is continuing since 2 yrs, I have been using moisturizers and frequent washing of the area but did not help. please recommend what should I do

Hi,

You said..you have itchy lesions on scrotum,buttock and perianal areas. Itching is more at night. And you suffer from 2 years. This much history is insufficient for comming to some conclusion..So, please answer the few questions....

- please describe thlesions..patchy,elevated,tough,reddish, scaly,crusted,oozing, infected, vesicular...etc
- besides scrotal,groin and buttock areas,do you have lesions elsewhere?
- is there remmision of the lesions sometime?
- when the lesions are exacerbated?
- which oral and topical medication you use?
- history of any systemic disease?
- past history of any disease?
- family history of any disease?
- your mental status... Anxiety,tension,depression,..etc

Please answer above questions. And kindly upload the photos of affected parts..so that I can guide you in managing your problem.

Hello doctor

No remarkable damage, small boils / cut like feeling on the are. Just irritating itchin,, quite embarrassing in public area.

No remission

No systemic disease

No family history of disease other than high BP.

Generally remain calm, no much stres..
Working hrs are Long.
To avoid itching, I take bath 2/3 times bath, but doesn't help.

Hi,

Welcome back to HCM...

And thanks for uploading photo. After reading history and seeing the photo, I feel that most probably you may be having intertrigo with scrotal dermatitis. You suffer from 2 years..that's why scrotal skin and upper inner thigh skin became thick and pigmented due to long term continuous itching and rubbing. It is good that you do not suffer from any internal disease like DM. Your working hours are long,which may cause stress resulting in scrotal pruritus. Intertrigo is an inflammatory condition of fold between upper thigh and scrotum. That may be due to fungal infection like Candida albicans. That might be exacerbated by perspiration, soap bath or frequent bathing for long time. You consult dermatologist for firm diagnosis. Still, I would like to suggest few tips for managing your disease...

- avoid frequent bathing
- keep the area dry
- improve hygiene
- take Sitz bath in water with few drops of dettol
- avoid soap bath
- take rest for few min. during working hours
- clotrimazole powder may be sprinkled after bath on dry skin in morning
- apply mild steroid with clotrimazole cream at night
- take antihistaminics like levocetirizine to get relief from itching
- take antifungal like ketoconazole for few weeks
- if needed, take steroids in tappering dose for few weeks
- take anxiolytic like fluoxetin in low dose
- wear cotton cloths

I hope above tips might be helpful to you. You might take drugs after consulting dermatologist.

I feel that you might be alright after few weeks treatment.

Thanks a ton, I remember one more thing which I forgot to mention, do apologize.
This itching many a times happen on the complete body, when I rub it, the area swells and become reddish for a while at times sharp white color rough surface devedevelopes on biceps or triceps with quite itchy for a while.

This is not regular but may be 5-7 times a month, especially after long day work or long play time.

May be this input help you with previous info.

You do not worry, the itchy rash may be allergic rash.
