Niacin,Topamax,Zetia,Gabapentin,Primidone,Tamulosin,Tacrolimus,fluconazole Flovent,folic acid, penicillin V potassium. methylprednisolone,omeprazole,potassium chloride, vitamin D
Medical issues - bone marrow transplant, rejection of esophagus by new immune system, essential tremor, high cholesterol, low urine flow
The pain began approximately at waist level and moved down slowly before disappearing. It was quite intense. It might have been from hydroxyhloroquinine sulfate, which I stpped taking Saturday. No fever.
I have never smoked. I tried alcoholic drinks, but they didn't taste good.
The incorrect sentence (sorry - I apologize) should have read "I have some sort of bump on my abdomen"
Seems like a colic, please revert with further information
Detailed Answer:
Hello,
Thank you for the follow up and clarifications.
I have a few more questions.
*why did you have an esophageal transplant? When was this?
*why are you on penicillin v and fluticasone?
*why were you on hydroxychloroquine? How long have you been on this?
*did you have any joint pains after initiating treatment for three transplant?
*were your cholesterol levels high before the surgery?
*how long have you had the essential tremor?
*where exactly is the bump on your abdomen? (below/above navel; left/right/center)
*what is the colour of your urine? any changes during the stomach ache?
*what is the colour of your stool?
*have you had any recent imaging studies for your abdomen and chest? Like ultrasound or MRI?
-how many surgeries have you had? Which parts of the body? When?
Your description of the pain sounds like an intestinal colic. Please get back to me with the above requested clarifications.
Regards,
Dr. Shuba Hariprasad
There was no esophagus transplant. Flovent +fluticasone + methylprednisolone + prograf are used to prevent rejection.
I am on penicillin because I was catching too many colds. The fluticasone is above.
The hydroxychloquine I have been on maybe 4 months, but it seems to be the cause of the high stomach pain.
I have had various joint pains and tendon problems related to methylprednisolone and the long (~ 10 year) I was on this drug Both of my rotator cuffs are torn, one worse than the other.
I don't recall exactly the cholesterol levels, but they are always perhaps 50% above the desired range -sometimes less, sometimes more. I just received a notice that they were all okay!!
Essential tremor for maybe 15 years, bone marrow transplant 10 years (both approx.)
Bump is navel height, and to the left
Stool - brown continuously, except yesterday they were veey large
Urine - Yellow
No recent images of chest, bot I expect to get one Wednesday
Surgeries - Appendix tonsils when I was ~ 5 yrs old
Shoulder for rotator cuff
Many (~15) endoscopies to figure out the problem with rejection of esophagus and opening up the esophagus.
Thank You,
XXXX
To check for impacted stools, or tumor
Detailed Answer:
Hello Mr. XXXX,
Thank you for the clarifications.
I believe you may have had a transient partial obstruction of your bowel, possibly causing intussuceotiin which has cleared up mostly, thus relieving the pain. The lump could be impacted Fecal matter.
A tumor and diverticulitis needs to be ruled out as well
*does the lump feel hard or soft? Is it painful or sore? Does it move around?
*when you had the intense pain, were you able to pass stools and gas? What color was the stool?
*have you had any colonoscopy?
*why did you have the bone marrow transplant?
*when you say esophageal rejection, do you mean that it closed up and you weren't able to eat of drink? Did they say it was due to the bone marrow transplant or the Medications taken for it?
I recommend :
-a gastroenterologist opinion as you have planned
-a CT abdomen to check the lump.
-complete blood count.
hydroxychloroquine can cause loss of appetite & gastritis. Take bland and fluid diet like soups, fresh juice, poridge. Keep sipping water through the day.
Hope I've helped. Please let me know if there is anything else I can help you with.
Wish you good health

Dr. Shuba Hariprasad
The lump feels soft and it seems fixed in location. It is not sore or painful.
Yes, when I had the pain I was able to pass gas and normal brown stool.
I was going to get a colonoscopy in May with the doctor I contacted and will be seeing tomorrow. I have had a couple already some years ago.
I had the bone marrow transplant for myodysplasticia (approximately). I was treated for CLL with some cancer drugs and suddenly I couldn't walk upstairs without being winded. The stem cells were from my sister.
Yes, the esophagus closed up and it was hard to swallow food. Once I got food stuck and a doctor had to pull it out. I never had trouble drinking. They said it was due to the new immune system attacking cells in the esophagus - nothing to do with medications.
Recently I have been very thirsty, and drink a lot of water, but should probably drink more.
Thanks,
Hal
To check blood glucose, electrolyte & liver function tests
Detailed Answer:
Hello Mr. Hal,
Yes you probably should drink more water.
*have you sugar levels been normal?
The lump needs to be checked out. It seems unlikely to be impacted stools as you were able to pass normal stools during the time of pain.
As you will be seeing your doctor tomorrow, a clinical exam will help narrow down possibilities (lipoma, lymph node, hernia?)
I also recommend checking your blood glucose, liver function tests and electrolyte levels.
Don't drink too much plain water. It will lead to low sodium and cause neurological problems. Have soup, tender coconut water, as well to balance out the salt with the water.
Hope I've helped. Please let me know if there is anything else I can help you with.
I would appreciate a follow up after your appointment with your doctor tomorrow.
In any case,
Wish you well and hope you feel better.

Dr. Shuba Hariprasad
I had the endoscopy today. The doctor described a yellow bile in the intestine. Sind bleeding, and most everything was red. Samples were taken various places and are expected back in a few days. The doctor told me that my symptoms matched the observations in the stomach, and he believes this is some bacterial infection.
Thank you,
XXXX
Helicobacter pylori infection?
Detailed Answer:
Hello,
Thank you for the follow up.
What is 'sind bleeding'?
*Did he say you had stomach ulcers and it looks like Helicobacter pylori infection?
*what was lump on examination?
I appreciate you keeping me in the know. You don't need to reply to this unless there is something else I can help you with.
Thank you and hope you feel better and recover soon.
Wish you good health.
