Acne, as you must be aware is pretty common condition caused chiefly due to hormonal influence and clogging of pores and followed by infection and inflammation.

Thank you for the pictures.
I can see there is lot of acne and pigmentation over back.
The treatment you are currently taking seems okay but not complete.
Continue the oral medications as you are taking right now.
Mix and apply Zyclin nano gel and another gel containing Azelaic acid 20% ( Aziderm gel) twice a day.
Apply a gel containing adapalene and benzoyl peroxide at night time ( Epiduo gel)
Use medicated bar like Medsoap acne bar or Acne aid bar over affected areas while bathing.
Follow the above regimen for 1 month and follow up with clinical pictures.
I am sure you will be fine soon enough.

Someone told me the acutret is too low at 20 mg for my weight which is 72 kg .
saying provided my blood test is fine i should be on atleast 40 mg of acutret.
Also recently i purchased neutrogena body scrub which has salicylic acid . Is tht ok ? or should i use a medicated soap?
Basically i feel my back isnt really drying up but my lips and face are getting dry so isnt the back also supposed to get dry?

Acutret contains Isotretinoin, which works very well for acne.
Considering your weight we can definitely increase dose to 40 mg daily but such high dose can lead to excessive dryness of skin, mouth, eyes and also can dead to increase in bad cholesterol levels.
I think you can try using 30mg Acutret once daily at night.
For dryness associated with it, use good moisturizer like Cetaphil moisturizing lotion and use a lip balm frequently.
Salicylic acid based body scrub can be used. Actually the main ingredient of medicated soap I suggested you is Salicylic acid only.
Make sure you add Epiduo gel in your regimen for better results.
Follow up after 1 month of treatment.
