Hello,
Thank you for using HCM to post your health question.
I read your question carefully.
I will need some more information about your question, because it is not clear how can I help you.
For how long you have had the pain? How did it started? For how long have you been taking Tramadol, and is it helping you?
What do you mean with your toes are curling over? Please give me as much information as possible, because otherwise I won't be able to answer specifically the question you have.
Hope this is helpful.
Looking forward to your follow-up.

Hi been on tramadol for months had back apin for a while just getting worse and my toes are bending over if that helps
Brief Answer:
you will need to see your local physician so he can examine the new symptom
Detailed Answer:
Hello,
Thank you for your follow-up question.
From what I understand, even if you are taking a very strong medicine, for a long time, you still seem to be getting worse.
Tramadol on the other side is not a medicine that is used for longer times.
The bending over of the toes is a new symptom that may or may not be related with the back pain.
So summerizing all those symptoms above, I would say that the best thing to do is to consider doing a visit to your local neurologist, so he/she can examine you and evaluate the changes and new findings, and do the necessary labs and imaging studies.
Since Tramadol is used for a short time, your doctor may need to switch you to another medicine, or a combination which will be helpful, allow you to be free of pain, and also have less side effects.
Hope I have helped you somehow.
Let me know if you need more clarifications.

What woild be the bezt medicine for pain
Brief Answer:
There are many medicines. The action will depend on your problem.
Detailed Answer:
Hello,
There are many, many medicines for the pain. All of them are helpful. If one medicine would work or not depends on many factors, and many of these factors are individual.
Like for example on your case, the fact that tramadol that you use 4 times/day is not working, is a sign of tolerance from the medicine. Tramadol is one of the strongest medicines for the pain, but is not used for back pain, or chronic pain. If you use Tramadol for more then 1-2 weeks, then it will give dependence, will will not start working as much as it used to work.
You should not regulate the dose of Tramadol yourself, nor stop it immediately. You should contact a doctor, who will change the medicines you are using gradually, until he finds the medicines that are more suitable for you.
Hope this is helpful.
