Suggest Treatment For Severe Depression While On Zoloft

Question: Hello Is it normal to have dark thoughts when starting new medications and does it go away. Is there something wrong with the medication and or do you think the medication causes these? I went through a depressive mood a few months ago like you know but I felt like crap but don't remember getting the those thoughts at first. I figured I would try Zoloft at 50 mg because I figured that is what they will start me off and within the first few days I had the thoughts. My question is I think the Zoloft had this affect but not sure because every time I started it this seem happen in the past. I probably should have started at 25 mg. I did it for only a few days because it made me feel like this. Now it seems no matter what they wont get out of my head. They go away and then come back throughout the day. I know in an earlier thread you said to give the lamictal a chance and after a few weeks you will know if its right for me. Now my question is that at therapeutic level or overall. I have been on lamictal for 5 weeks overall and 1 week at 150 mg. It seems to help my mood but the thoughts don't bug me all the time (enough to really scare me). What do you think? Im waiting for a appt with the psychiatrist early next week if not tomorrow. By next Thursday they will put me up to 200 mg of lamictal.

I think it is difficult to say whether it is normal or abnormal because scary thoughts beyond a certain limits can not be considered normal and they are part of disease and disease is never normal.

Now regarding the phenomenology there are two explanations offered.

First they explain that when you have severe depression your thinking process is limited or better to term clouded. Although thoughts remain there but brain have no capacity to bring them to conscious mind. Once the antidepressant started the brain gets more energy to deal with these scary thoughts and brings them to conscious mind so they can be dealt appropriately. In other words it can be said that antidepressants help the brain to deal with these dark thoughts.


The second explanation is related to progressive nature of illness. The scientists offer that the disease process leads to development of symptoms slowly and bad thoughts may be at the another end of ladder. Over the time person gets one symptom, than another symptom and so on. As we know antidepressants take time to act sometime the individual feels that the thoughts were brought by the new medicine because of some perceived temporal relationship between them. Infact this is delay in action of antidepressant action which makes development of such associations.

To conclude it can be said that even if these thoughts happen the antidepressant is there to help you and there is little possibility of worsening them due to use of medications.

Regarding lamictal, as you said the mood part is quite good and it can be hoped that thoughts will be dealt similarly with continuation of medication. I think one week of effective dose is not that much sufficient and we can expect further improvement over course of time.

I guess they always put it can worsen when starting these medications. They say to tell your doctor if that happens. I just think if i would have started at lower mg on zoloft or never took it and went to the lamictal these thoughts may have never started. Im not sure. I was trying to take it for the depressive mood. My issue is once it comes to my head once its hard to ever totally get rid of it. And this scares me.

I understand your speculation but it rarely in the way you think. They put it there because patient never gets to know that he may develop these scary thoughts and doctor never tell this on most occasions feeling that the treatment process may not be started due to fear of scary thoughts.

But I must assure you that like the depressive mood, the dark thoughts likely to go away with continuation of treatment process.

Sometime therapy also helps in this regard and if the scary thoughts continue, the option of therapy can be explored.
