He needs to be assessed for neck stiffness, signs of meningeal irritation and focal neurological deficits associated with these symptoms. Based on this it could be decided whether to manage him conservatively or advise further investigations including blood counts, CT scan , lumbar puncture for CSF examination and culture. Based on your description of the symptoms, diabetes is unlikely though blood sugar can be done to rule out a markedly elevated blood sugar. I would advise you to take him to an E.R near you. You can give him tylenol or ibuprofen for now.
