Has she started menstruating? is this pain related to periods?

Hello,
Thanks for posting in HCM.
Usually young adolescent girls can have pain in stomach when they just start menstruating or during the periods. As body is getting used to these new changes it is common to see this problem.
So I want to know whether the pain is related to menstruation or not?
Has she already attained her periods or not yet?
Meanwhile rather than giving her paracetamol and ibuprofen, give her antispasmodic meds like meftal spas / spasmindon/ spasmiproxyvon. If these are not the generic names in your place ask for antispasmodic containing dicyclomine or hyoscine. It should help her a lot by reducing the pain.

Hello,
Thanks for following up with me and answering the questions.
So to summarize she has this ongoing pain in stomach and also headache which never comes down.
There are 2 possibilities as I can see this. I am guessing on what history and information you have given me.
1. She has migraine which is seen in this age group and is quite worse variety. She will need to see a neurologist for the workup and detailed history taking and clinical examination to arrive to diagnosis. Medications are there to take care of this problem and a large number of girls do have this problem.
2. She could be having some sort of gastritis either erosive type or it could be ulcer as she must be taking this ibuprofen from long time which causes irritation in stomach causing gastritis and also ulcer. I would like to advice here to start her on tab pantoprazole 40mg once a day in morning half hour before breakfast. Try to avoid ibuprofen and reduce the pain with paracetamol only. If required tramadol can be used. Ibuprofen is NSAID and it can cause gastric problem as said above. Avoid spicy food for time being. As you are seeing her consultant on Thursday, with above advice you have time to see whether it works and is really the cause as I guess it could be.
