Suggest Treatment For Severe Itching On Scalp And Severe Hair Loss

Question: sir,
i have severe itching on my head and severe hair fall i had consulted doctors before writting to you they said that itching is due to dandruff and have given some lotions to me for dandruff reduction but i have severe itch even after no dandruff condition also
doctor suggested me ketafung, candid tv lotion or shampoos whatever
after taking bath with i just relaxed only on the day i took bath and it is pushing to take head bath twice a day
if the condition is severe i have like burning sensation and like pimples on my head when my head is wet severe itch than dry same problem with high sunlight
i am suffering wit h these for almost 5 years i just want only an itch free head

Therefore I would like to know whether are there any visible bumps or pimples on scalp? Or is it only the feeling that bothers you?

Moreover dandruff usually does'nt cause bothersome hair fall though it does increase hair shedding.

Approximately how many hair do you lose per day? (100)?

Is the hair fall more from the top and front part of the scalp Or is it diffuse? and is it also associated with thinning?
Is there any family history of early balding?

1.Yes sir my dandruff has improved after using the anti dandruff shampoos
2.When i got this problem i was in hostel
3.Sir i forgot to mention this my scalp becomes oily within a day of head bath secretion of seborrhoeic glands s very high
4.I didn't observe any bumps but there are small pimples. no early balding in my family
5.Hair loss is more or on top and front.yes sir thickness of my hair is reducing when i rub my hair i use to see 10 to 20 hair which i don't see from normal people that makes unusual
6.Sir i have some abnormal sensation but not pain

Hi.

Thank you for the additional information.
Now, that you dandruff is under check you may still continue with a ketoconazole based antridandruff shampoo as part of a maintenance treatment for dandruff and oily scalp.

Ketoconazole is antifungal, antiinflamatory as well as sebostatic i.e inhibits oily production.

Besides, for excessively oily scalp an Oral Retinoid e.g Isotretinoin may be added. Isotretinoin is sebostatic; it shrinks oil glands to 10% of the size within a month. That should also take care of small pimples that form in your scalp.

For hair fall I would suggest that you add a Biotin supplement once daily. Biotin is also useful in seborrheic dermatitis Or dandruff.

The abnormal burning sensation is most likely explained by scalp dysesthesia. A tricyclic antidepressant e.g Amytriptyline, twice daily Or Pregabalin (usually in combination with Methycobalamin), twice daily would be useful.

These are all prescription medications, therefore, I suggest you to talk to your dermatologist for the needful.
