I am sorry to know about your problem. It has been your third round of medication which is not making any difference so here are some suggestions which i can give you.
Yes you should go back to your doctor and Ask him about further possible strategy and treatment options you can put to your use.
Few options which i can tell you are as following , you can discuss them with your doctor and then you can decide how to move forward.
1) Change the antibiotics:
There is always an option to change the antibiotic. There are many antibiotics in the market which are more potent and have been proved to work well for ENT infections, you have been taking antibiotics for quite long so may be the bug you have this time is resistant to the antibiotic you are using at the moment.
Change of antibiotic may work wonders for you.
2) Get a culture sensitivity test done:
This test will tell you will bug is causing this and which antibiotics are killing it, then all you have to do is to take that particular antibiotic and get targeted killing of that germ this is expected to help you big time.
3) Complete this course of antibiotics and see the results:
You can complete this course of antibiotics and consult your doctor after this.If things don;t settle down.
4) Meet a specialist.
If you haven't visited a specialist yet then meet an ENT specialist.
5) Consider other diagnosis:
Discuss with your doctor if he could me missing any diagnosis or any other cause of the problem. If there are any tests or scans he might wanna get?
I would suggest you that if you are not getting any improvement then visit your doctor and discuss with him the suggestions i gave you and make further action plan after discussion.
