Brief Answer:
Supportive care

Detailed Answer:
Thank you for asking

I read your question and i understand your concern. No doubt with any chronic illness , clinical depression is inevitable and needs supportive care and psychotherapy sessions and alot of will power to fight this, so before answering your concerns i would suggest to be brave and hope for the best.

Shortness of breath, loss of stamina, dizziness upon standing and losing balance with normal otolaryngological work, normal vestibular evaluation, normal heart studies can be attributed to too much meds you are on, almost all of them can cause these symptoms , supper added hypothyroidism likely inadequately treated and your recent mentioning of skipping thyroxine dose adjustments might have played a role. Complete thyroid hormonal evaluation needs to be done to assess thyroid status and these exertional symptoms need to be managed supportively as these medications can lead to such discomforts you mentioned. Also rheumatoid arthritis on its own affect heart functions and slower it down as your stress test mentioned raising blood pressure, explains the shortness of breath and stamina issues , also at age of 74 age related structural functional decline is also a fact and can not be ignored.

Depression needs frequent sessions of physiotherapy, vocational recreational rehabilitation and compliant use of antidepressants. Thyroid hormonal evaluation and mobilisation as much as possible with least possible doses of immunosuppressants for rheumatoid arthritis will help. You need a joint consult of local rheumatologists, endocrinologists, psychiatrists and nutritionists to make a complete management plan which you can follow and abide by for better outcome of this multiple diseases presentation. I wish you good luck.

I hope it helps. Take good care of yourself and dont forget to close the discussion please.
