Brief Answer:
You need to get your pulmonary functions tests done

Detailed Answer:
Hello,I am Dr.Rajesh Teli,MD.
I have gone through your query.you may have few causes of exertional shortness of breathlessness.
1.long standing chronic bronchitis may have weakened your lungs and it is causing your symptoms.Pulmonary function tests will help us to establish this fact and plan further line of treatment.
2.As you have severe obstructive sleep apnea and not using Bipap or Cpap ,you may have developed pulmonary hypertension causing symptoms. My suggestion is to use ventilator on daily basis.
3.As you are diabetic and were smoker ,we should rule out ischaemic heart disease .For this you should get ECG.
Echocardiography too will be useful for decision making in all three conditions we discussed
I suggest you to get these tests done,
my best wishes
