I have gone through your history and i might say as follows:

Since you have fallen 3 weeks ago (depending on the way that you fell) probably you have muscle strain (pulled muscle). A pulled muscle may be able to return to normal activities within three to six weeks or sometimes even more. It depends from extent of damage that the muscle suffered.

If the pain persists for more than 2 weeks (in your case 3 weeks) muscles lack of activity leads to disuse atrophy (muscle wasting) and weakening.

Now the following measures might help your body (muscles) to recover faster :

-massage your muscles in the shoulder area gently
-don't do heavy exercises on your shoulder. Just gentle moves of your shoulder. Since your pain is persisting, that means muscles haven't healed properly, so you should not put stress on them (a temporary holding cast/sling will help).

-use round the clock analgesics eg. Paracetamol (650mg thrice daily after food for another 4-5 days) / Ibuprofen(400mg twice daily after food for another 2-3 days). It helps with the pain and the inflammation of the muscles. Applying heat in the area might help the muscles recover faster.
