Suggest Treatment For Singular Nodular Acne On The Face

Hello

I would like help from a dermatologist please preferably dr kakka if available


I have lesion on face since Sunday. I think singular nodular acne

It's a reoccuraNce of something I had 4-5 weeks ago that took ages to resolve and scarred a bit. (See picture)

It is more under the surface. I can also feel lump of flat pea size quick is very tender. It's a bit red (see pictures) but there isn't much else to see colour wise , it's not angry red but face is a bit puffy & Tender for about 3cm diameter. Tender to wash
Area of face feels numb, swollen very tingly all the time and tender


I spoke to a GP last time and she said to leave or take Antibiotics but to leave initially and I did and It went down alot within a week but took a long time to resolve,

But I think this time is worse . I think now it is probably a form of nodular acne. I think this because of the tenderness, not visible inflammation around the area, the fact a lot is clearly going on under the skin and the fact is tingling, face feel s like being eaten which I assume is nerves being affected.

But It does not visually match how it feels so I'm not sure how bad it will be considered.
I enclose pictures which show the scaring from its previous time and also these pictures show the area after its been washed today ie at its worst

. Treatment wise, i have been applying differin daily and zindaclin gel most days to the area it now feels irritated.


NB I have a history of singular bad acne lesion , usually connected to my period.

I scar easily which is my concern here.

I am bedridden with CFS so can not visit a dermatologist in person.
My GP can not prescribe hormonal meds like the contraception pill because of my immobily.
He has refused to try spironolactone because of my severe ill health.

I self manage my acne prone skin with topical differin daily , that is all.

My question is should I be asking for antibiotics for it? Is it a bad enough nodule for this and as it is a singular lesion is it appropriately action? I live in uk where drs do not do steroid injections into the site

Please could you advise

Dear XXXXXXX Welcome. I have reviewed the recent images.

For just a single papular/ nodular acne lesion such as in this Image I would suggest you to apply a topical antiacne preparation e.g benzoyl peroxide 2.5% gel, twice daily, to the affected area (spot application). This should be enough.
You may also take an oral antiinflammatory e.g ibuprofen or diclofenac, once or twice daily for symptomatic relief from pain and swelling. I don't think you need to take oral antibiotics at this stage.

Hello Dr Kakka,

Thanks for your advice.

I have been taking ibuprofen regularly.

Unfortunately over past days the lesion has continued to grow and is much more noticeable and a solid lump in skin, i have uploaded 3 pictures to show this. Can i just check your opinion is still topical treatment and that it isnt a more serious nodule in your opinion?. Im afraid its going to turn into one of my scarring nasties.

Can take an oral antibiotic (Azithromycin) for 5 days

Detailed Answer:
Hello. Thank you. I have reviewed the Images.

You can take a tablet of azithromycin 500 mg, once a daily for 5 days, if the acne nodule has been growing and does'nt seem to be improving with just topical antibiotics.
In addition to this you may continue with topical antiacne preparation, preferably benzoyl peroxide 2.5% gel, twice daily.
