Suggest Treatment For Sleepiness And Fatigue After Hip Implant Surgery

Question: My father age 86 has undergone hip implant operation. However after operation he feels listless, sleepy throughout the day and does not want to take even physiotherapy session. He has mild sugar and normal BP. IHe was given sodium by doctors. Is the excess sleepiness or fatigue due to aneshtesia hangover. (he was given spine anesthesia during hip implant). He is taking normal diet with sugar free meal (his sugar after meals is 130) but likes to rest on bed.
Will extra vitamin /mineral supplement is needed in his food like amway nutarlite. What specific test are required to know the reason for his fatigue/sleepiness.

I am Dr. Prakash HM and I will be answering your query.
Based on your query, my opinion is as follows:
1. The surgery was at least more than a week back. Anaesthesia effect can last a maximum of three days. Beyond that reduced activity, listlessness or increasing sleepiness could be due to other causes.
2. In the elderly, cognition reduction is severe, especially during alien atmosphere. He was hospitalized for few days, during which his cognition has reduced.
3. Improvement in cognition would take already a month, as he gets used to usual life. He will require support from family and involvement in social activities along with mild exercises is necessary.
4. Yes, protein and vitamin diet through amway or any company can be provided. Fiber rich in diet composed of fruits and vegetables, along with adequate hydration (at least 5-6 liters of water/day) is necessary.
5. Sleep time needs to be fixed. Night compulsory 9-6 and daytime, maximum of hour. At other times, involvement in other home activities, even mild is necessary.
6. Hope his hemoglobin and thyroid hormone levels are normal. If not done, can be tested once. Blood sugar is under good control and need to continue the same. Wound care is important. Physiotherapy will require more coercion as it is very necessary. He would not understand much for now, but need to be understood slowly.

Psychological support from surrounding friends and family is necessary at present. If not supportive now, further deterioration can occur. At present, it is reversible.

Dear Doctor
I have got blood test of my father done through thyrocare. He was operated two months ago. But after operation he has not been normal. General laziness, sleepiness, lack of interest, demnetia symptom and depression symptom. He one fainted also.
His blood report is showing several difficency. TSH seems to be very high. High sensitive C reactive protien marker also abnormal. Please comment on the blood report urgently. Was high thyoride resposbile for fainting.
What medication must be started and what may be side effect.
Blood report is attached.
Any suggestion for referring to specialist.
He is most of time lying on bed and difficult to take him to hospital as we are in XXXXXXX and he is in XXXXXXX with my sister family and a male nurse.

Based on reports, my opinion is as follows:

1. TSH is high, indicating hypothyroidism. It could be responsible for most of the symptoms. An endocrinologist evaluation and start of thyroxine medication is necessary.
2. He also has mild anemia, which could be an additional factor. Various nutritional values are also low, and he will require good nutritional supplements.
3. High Anti-CCP indicates ongoing inflammation. Need to control thyroid hormone levels and improved nutrition. After that, it could come into control. Mild exercises, weight loss, good hydration will be helpful.
4. Other abnormal values, need not worry at present. Major is TSH levels.

Most important, to reduce TSH levels. Need to meet endocrinologist for further evaluation and treatment.
