Suggest Treatment For Sticky Nasal Secretions
Question: Im XXXXXX 25 years old from XXXXXXX I had sinus related issues since last 5 year. I had undergone sinus surgeries FESS+Septoplasty on first time and FESS on second time in 2010 & 2012 respectively. After the second surgery sinus issues relieved slightly but a special condition started. Through out the day i will have sticky post nasal drip which is different from mucous. And it will have bad smell. I become very sensitive to dust. Whenever i exposed to high dusty atmosphere the smell will become further bad and at that time even flies also get attracted to my skull may be due to the bad smell. I always feels itchy on right upper part of the skull. Some reddish pimples are developed on skull and neck when the pressure inside the sinus increases. Now a days its noted some descending of this discharge occurs through out the body and swellings noted on tip of fingers on both leg and hand, and lot of small pimples are forming at back side of the body. I use nasal sprays daily , without that its difficult. I feel sleepy through out the day. Due to these issues quality of life dropped drastically. I feel very hopeless. Please suggest me a solution.

Based on your clinical details, it appears to be recurrence of sinusitis secondary to allergic rhinitis.
Post fess surgery, on nasal sprays, confirms you are having allergic rhinitis.
sticky, fowl smelling discharge confirms active infection secondary to allergic polyp (itching) or recurrence of sinusitis.
I suggest the following measures to overcome your recurrence.
1. steam inhalation.
2. saline nasal drops 3 drops/3 times for easy clearing of nasal secretions.
3. Re-evaluate again with ent surgeon with CT scan of para nasal sinuses (allergic polyp, extent of sinusitis).
4. As such, allergy or sinus infection does not cause any swelling over digits of hand and leg. You require blood tests to rule out other causes.
5. presently, you require intravenous steroids, another course of antibiotic and prophylactic anti allergic combination like citrizine + montelukast daily one tab at bed time.
Kindly discuss with your treating doctor about these instructions and medications, as they require to take under medical supervision.

Sir, Please go through below points.
1. I don't feel any obstruction in both the nostrils, Sticky secretion is not associated with headache . on evening I used to have little bit headache at that tie discharge is not there. Morning time there is some pain on skull while touching. It relieves after this sticky discharge goes out. morning time post nasal drip is more.
2. I was regularly taking steam for last three years. But now days the day after steam taking I feel so energy less. Sinus pressure become more. The days on which im taking some exercises help me to drain heavy discharge or I have to involve in some mechanical activities. it doesn't come out naturally.
3. Before last three months I have done CT (PNS) followed by MRI of brain both of them found to be Ok.(sinus were free of polyps).
4. But the reddish pimples on the back of the neck and head comes only when ever my sinus health goes bad. Im observing this for last two years. Whether this kind of issue will be there in sinus patients. ? Or this is something related with last surgery. because this condition was not there before surgery.
5. For finding the route cause of swelling of digits what are the blood tests to be done, kindly suggest.
6. Continuous use of steroidal nasal sprays have any side effects?
