Suggest Treatment For Swelling After Tooth Extraction Procedure
Question: Hi, my dimple is gone after filling. It has been a week and my dimple didn't come back! I have very deep dimples since I was born. What could happen and will it come back or not? Also what can I do about ti? Thanks
Brief Answer:
It will certainly come back.
Detailed Answer:
Hi.
Thanks for your query.
Saw the picture, before the dental procedure.
For filling or any dental procedure, you must have been injected local anesthesia. This causes the edema. The procedure itself can edema.
It is this edema that can cause vanishing of the dimple.
Since it is just 1 week, the edema must not have settled yet.
Since you have deep dimples since birth, it will certainly come back.
You can take anti-inflammatory medicines, and wait till it will reappear.
I hope this answer helps you, please feel free to ask for further queries if you need to or if you feel that there is a gap of communication.
It will certainly come back.

Brief Answer:
Please post recent photo if possible, no need of Neurology
Detailed Answer:
I can certainly understand your concerns.
There is no role of Neurology.
Not seen such a case or heard so till now and since these are by birth, dimple will surely come back as good as was before once the internal edema settles.
So please stop worrying.
Please do not fondle the area as this increases the edema (if you doing to see again and again.
Your present photo would have helped me, please post if possible.

Thank you so much Dr. T Chandrakant
I uploaded 2 pics to compare.
One was taken now and the other one was a while ago before the dentist visit.

Brief Answer:
Better to check once for facial palsy.
Detailed Answer:
Pictures speak thousands of words.
Thanks for the comparative ones which show that the subcutaneous edema might have caused the dimples to vanish temporarily.
Although it does not looks like facial Palsy as you had once though of Neurologist, now looking at the picture, I think it is better to consult any Doctor like PCP and get the facial nerve tested. If it is found to have some paresis (very small partial paralysis) you may then consult a Neurologist for further evaluation and treatment.
I hope you must have already checked for the facial expressions yourself in mirror. How are they? Are they equal and normal; as I can see in the second photograph the angle of mouth look to be equally placed.
I hope this answer helps in decision making, please give me the feedback about facial movements, movements of the eyelids and the findings that the Doctor/s have given.

Hello again Dr. T Chandrakant,
I think my face is ok because my eyes and mouth are in their normal places. The only thing I can tell happened is my right cheek where the filling was is a little bit different like a very small swollen and the dimple is gone. I checked today with my dentist and he was so rude and his solution was "do not smile!"
I showed him your answer and he gave me 800 mg Iburrofen. I do not have any pain but he said this is the only thing that he can do for me. I checked with another dentist and they scheduled me to see the surgeon on Thursday. They think the dental hit my nerves on that side or something like that.
It all started after the filling. Do you think that visit injured me to the point that caused facial palsy?
My facial movements are normal except my right cheek. I don't have even lines on that cheek when I smile after the filling.
I'm so depressed and wish I could know what's going on.
Thank you for your help

Brief Answer:
Neurologist's opinion, nerve conduction study to see what the problem is.
Detailed Answer:
Thanks for your elucidate feedback.
The good part is that the eyes and mouth are in normal places.
You have been mentioning right cheek wherein the photo shows the problem on the left. (is it a reversed photo) Please check.
Your second opinion of another dentist looks more accurate, out of the 5 branches the buccal branch might have got neurapraxia due to the needle or injection.
Timely diagnosis and treatment can help to have complete recovery in most of the cases.
An opinion of a Neurologist with nerve conduction study will confirm this or rule out such a problem and will get an appropriate treatment.
Till then carry on the buccal exercises to keep the muscles working.
Once recovery occurs, the dimples may come back again.
I hope this answer help you.
Take care, Do not take stress or have anxiety. I know many want the dimples hence go for surgery and yours was so natural.
Please give feedback.

Hello Dr. T Chandrakant,
I just wanna let you know what happened in my last doctor visit. I went to a neurologist and he checked my facial neurons and he said nothing wrong with my neurons. He said your first opinion for me was the right one! I can see my dimple is showing a very little bit and I can smile better and my mouth looks in the middle, not like couple days ago.
Thank you so much for helping me and giving me the right answer even you didn't see me in person like the other doctors who I went to.
I'm glad that I found you and I will save your name in my phone.
Thanks again,
XXXX
