Imaging investigations are imperative especially if the pain and swelling is not responding to high doses of motrin. X rays AP , lateral and axillary or scapular views can be advised. Shoulder dislocation can also be picked up on CT if the x rays are inconclusive and suspicion is high. Rotator cuff tendon tears are also possible especially if the age of patient is greater than 45. MRI with contrast would be the ideal tool for diagnosis in that case. Further intervention and management would depend upon the findings in the imaging investigations. The arm should be stabilized and immobilized in a sling preferably in a mid prone , internally rotated position. Injectable NSAIDs like diclofenac can be given. Muscle relaxants such as tizanidine would also help.
