Brief Answer:
Rule out Cardiac as the first differential, Take him to a local physician

Detailed Answer:
Hello xxxxxxxxxxt, I am Dr. Mody and I would be addressing your concern.
On basis of the description you have given and the history my conclusions are as follows
1) What you describe is clinically called edema
2) The most common causes keeping his age , medical history are
Cardiac , decrease in hemoglobin or protein - albumin , kidney or liver related .
But based on the description and particularly I'd the swelling occurs more when he is standing , towards the end of the day and decrease when he goes ro sleep , cardiac should be the 1st thing I would like to rule out .
The most common reason is due to decrease in the pump of the heart or impaired relaxation of the heart .
I would strongly recommend that you consult your doctor and consider doing the following tests .
2 D echocardiography of the heart ,
Complete blood count, S. Albumin, Thyroid profile, urine routine for any protein loss .
Based on the above test results one can start a medication for the same .
Hoping this would help , however there is a strong suggestion to get him examined clinically by a local doctor .
Regards Dr Priyank Mody
