Suggest Treatment For Symptoms Of Anal Fissure

Question: I asked this question earlier but felt I didn't give sufficient information.

A couple of weeks ago I experienced some moderately severe rectal discomfort/pain.. Bowel movements were normal, no blood in stool. The discomfort gradually subsided to the point now of being pretty much gone. But there is still some rectal tenderness and discomfort, as well as some blood, again not in stool but on the toilet paper, as well as what I can only describe as small stool buildup over the course of the day around the rectum -- not discharge but buildup. I've noticed being in a hot shower noticeably relieves the symptoms.

I can understand your concern toward's these symptoms.
I read your previous query and i agree with previous physician.
1) Your symptoms are due to anal fissure. This is a linear tear on inner wall of lower most part of anal canal. When this tear is new it look like pink strike which is termed as acute fissure. Once tear starts healing naturally with process of fibrosis then it become chronic fissure.
2) Most common causes for development of fissure are either constipation or passage of hard stool or both. These conditions require more straining to pass stool which can cause breach in continuity of inner layer of anal canal and lead to linear tear.
3) Fissure cause pain in anal region due to passage of hard stool through linear tear. Same is also responsible for rectal bleeding on toilet paper.
4) This condition does not require any specific test or investigation to confirm the diagnosis. Simple physical examination by general surgeon is sufficient.
5) Treatment part consist of prevention of constipation, treatment for pain. Thus stool softeners, laxatives, analgesic medications for pain, ointment of anesthetic substance (to apply over anal region for quick relief from pain), anti-inflammatory medication to reduce swelling in acute fissure are mainstay treatment part.
6) Just to be on safer side, you should check Hemoglobin level, if you have bleeding episodes since long time

Yes, fiber-rich diet would help for this. Also you can try stool softener.
Main principle of treatment in anal fissure is to avoid straining to pass stool and to keep consistency of stool soft. These two would allow fissure to heal naturally.

I'm sorry to be any trouble but may I ask just one last question? I've noticed in reading about anal fissures that pain seems to be a prominent symptom, but apart from the initial pain a couple of weeks ago, I've been free of pain for at least a week now. Just have the discomfort and a sort of sense of minor swollenness. I know symptoms can vary from patient to patient. Would this still point toward a fissure?

You are right regarding your observation.
1) Acute anal fissure is always associated with pain.
2) Chronic fissure (already healed or in healing phase) are painless due to growth of fibrous tissue over mucosal tear.
3) Foreign body sensation with sense of swelling indicate presence of hemorrhoids also along with fissure. This hemorrhoids must have been developed due to straining more to pass hard stool.
4) Thus theoretically, fissure and hemorrhoids can develop together as they share common risk factor.
