Question: I have a synovial facet cyst in lower back, I have mild pain on right leg waist down to ankle almost whole day by standing ,do I need surgery? Paracitimol Panadol no help. What is a success rate for surgery and who is a good surgeon with good success rate.

A synovial cyst that is compressing the nerve roots is best if surgically removed. It will decrease the pressure on the nerves and the pain will reduce. If the pain is only mild then you can first try with pain killers and muscle relaxants. If the pain is relieved then further physiotherapy can help in delaying the surgery by a few years.
The success rate of the surgery is more than 70% provided the surgeon is good one. You need to consult a neurosurgeon in this regard for a proper assessment.
