Suggest Treatment For Tenderness And Swelling In Breast While Having Nexplanon Implant
I have the Nexplanon implant. About 2 weeks ago I experienced breast tenderness and swelling that lasted for about 7 days. Yesterday I started to experience cramping and have had some light brown spotting. I took a hpt and the result was negative. Is pregnancy possible? When should I take another test?

I understand your concerns.
1. When using Nexplanon implant, usually the periods are irregular or might be stopping. Can you please describe about your periods until now: are they irregular or no periods at all?
2. The breast tenderness and swelling you felt 2 weeks ago are suggestive for possible ovulation period. Everything you experienced is due to hormonal changes during this time.
The cramping and some light brown spotting you experienced yesterday are suggestive for possible monthly period. It is common to experience light spotting when having the implant in; so, nothing worrisome. Furthermore, your home pregnancy test was negative; so, no pregnancy.
3. With regards to "when to get the next test"; I need to know about your periods first, then, give a more precise answer.

Thank you for your reply.
1. I had the implant inserted on April 24th when I had my period and I have not had a period since then.
2. I guess I didn't realize ovulation was still possible with the implant. I didn't experience breast tenderness and nipple swelling to that extent before the implant. Can ovulation symptoms change with the implant as well?
3. I don't know when my period would be due but the last time I had intercourse was on July 28th if that helps determine when a test could potentially be positive.

Thank you for providing the requested information.
1. If you have the implant inserted April 24, and you have not had a period since then, then, all you are experiencing, I still think, it's due to incoming periods. Please bare in mind that the periods become irregular during the first year of implant insertion.
So, if you did not have any period since then, it does not mean, you are not going to have in the next months. The symptoms you are experiencing are suggestive for periods although the intensity and color of bleeding is not normal.
2. Yes, ovulation periods can also change with implant due to hormonal changes. Please take note that Nexplanon implant does contain an hormone called etonogestrel. One effect of this synthetic hormone is to stop ovulation. As it needs time to fully accomplish this function, the first year, the periods are irregular until no periods at all for the next 3 years.
3. If you had intercourse on July 28th, while this time no periods at all, an hpt was negative; then, I think it's your periods.
You can repeat testing in 10 days time. If still negative, and the symptoms will repeat or aggravate, then, can repeat testing and consult further with your OG. We should rule out possible change in position of the implant or any other damage.
