Hi XXXX,
Thanks for being on healthcaremagic.com.
I am Dr.Ajay Panwar,a neurologist,here to answer your query.

I understand your concerns and I shall do my best to address them well, however,I need to know some more details about your symptoms-
1)Since when you are taking Metoprolol and Warfarin? Is there any recent change in medications or dose of medicines?
2)Do you have other comorbidities like Diabetes,Hypertension or other illness(except replaced mitral valve) as you mentioned?
3)Have you been under stress or anxious for past few days?
4)Is the tingling in feet and fingers persistent or improves intermittently? Is it worse in day or night time?

I shall be glad to have you in follow-up with details so that I can answer this well further.

1. Been taking warfarin or other anticoagulant since 1998, metropolol for about 6 months. No changes in medication
2. Last blood test showed only a slightly high bilirubin level, probably due to hemolysis. Glucose, liver values, cholesterol wer all good.
3. Had a hectic schedule in February and March - 18-day business trip followed shorty thereafter by a 13-day trip. Had a bad sinus infection (viral) April 7-10. My job is constant high stress - nothing new.
4. It's become more intense in the last two days, adding the nose and other parts of the face. It's worse during the day.

Brief Answer:
Observation/ Fasting blood sugar/ Mental relaxation exercises.

Detailed Answer:
Hi XXXX,
Thanks for being in follow-up and providing further details.

As it's just 5 days of tingling of fingers and feet,I would not like to directly jump to neuropathy,as the diagnosis although, peripheral neuropathy can manifest like this in the early stages. However,stress/anxiety can commonly precipitate such symptoms and so for now,I would suggest that you should start doing regular mental relaxation exercises like meditation/yoga and observe for the improvement. Beating the anxiety,is likely to improve the symptoms.

Besides,Diabetes being the commonest cause of neuropathy,I would suggest that you should get a repeat 'blood sugar fasting and post prandial' levels done.

If the symptoms do not improve, further testing including nerve conduction studies can be planned.

Hope that helps. If you have some further questions,I shall be glad to have you in follow-up else please close the thread, rate it and write a review.
