Brief Answer:
He seems to have Enlarged Prostate

Detailed Answer:
Hello
Thanks for your query ,based on the facts that you have posted it appears that your grand father has Acute Retention of urine and he has been put an indwelling catheter to drain his bladder and now passing blood through catheter .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Since how long he has been catheterized ?
2) Have you done his Ultrasound Scanning of abdomen if yes upload report
3) Does he have any other major health issue ?
4) Have you consulted Urologist?

The cause of retention of urine at this age mostly due to Enlarged prostate and hematuria that he has is due to foreign body reaction of the bladder to catheter what is called as Catheter Cystitis .

Consult qualified Urologist for assessment and get basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.
4) Renal function tests

In the meanwhile start broad spectrum antibiotics like Cefotaxime along with urinary antiseptic like Nitrofurantoin twice daily and urine alkalizer thrice daily .Later on switch on to appropriate antibiotics as per culture report.

Further investigations if required and modality of treatment will depend upon result of these tests and final diagnosis.

Hope I have answered your query ,please feel free to ask if you have more questions , I shall be happy to help you .
Thanks and Regards.
Dr.Patil.
