Suggest Treatment For Vitiligo
Question: I am suffering from active vitiligo and spreading rapidly.Earlier tried oral steroid(zempred, defcort),immunomodulator (levimasole) but it gets controlled while i am on above mentioned medication but start to spread in more severe form once the medicine is stopped.tried herbal and homepathic as well but it is of no use.Tried NB-UBV but it does not work when vitiligo is active.Recently tried immunoral tablet but could not continue as it is causing too much stomach upset symptoms like stomach burning, nausea, burping even after taking it with PPI drug like rablet-l or pan-d.So again I am prescribed tricort injection as it is spreading.
Not sure about the side effect of frequent steroid dose as i am taking steroid for last 2-3 years .Now for last 3 months I did not take steroid so it is spreading rapidly.I am very much worried as it is not getting stable. Please advise is it possible to make it stable for long term. is it curable.should I take tricort injection again or any other medicine is preferable

Brief Answer:
Treatment needs to be continued
Detailed Answer:
Thanks for asking on HealthcareMagic.
I have gone carefully through the details and understand your concern. Vitiligo is hard to treat and is not curable. However, with proper treatment the situation may be kept in control and long term stability can usually be maintained. Treatment with oral and topical steroids (zempred, defcort) as well as immunomodulators (levimasole) in addition to UVB therapy needs to be resumed and continued. I do not think that injectable steroids like tricort are needed.

Thanks for the response.I have below query:-
1) I am worried about long term steroid dose side effect as already it has been 2-3 yrs i am taking steroid. I got high cholesterol earlier now it is under control. can it be caused by corticosteroid. How long can i continue this steroid treatment without side effects. Is there any safer option other than steroid that can make my vitiligo stable.
2)Not sure why doctor prescribe me tricort injection this time instead of oral one.Is it safer than oral steroid? Please advise.
3) I was taking some herbal ashwagandha preparation earlier that was provided to me by an ayurved doctor as immunomodulator and stress reliever although it did not work that much. Can i consume that with oral corticosteroid/immunosuppresent or is there any drug interaction between them.

Detailed Answer:
Thanks for writing back. Let me try to address the issues:
1. If taken in proper doses, long term steroids would not be a cause for concern. However, you need to get your blood sugar levels periodically checked as steroids can elevate the blood sugar and can indirectly affect cholesterol levels. If your cholesterol level is raised you need to take statins.
2. It is safer than oral but the specific scenario needs to be considered. Take the injection and see whether it helps.
3. No specific interaction is known. However, efficacy of these ayurveda medicines would remain in question.

Brief Answer:
You can go ahead with tricort

Thanks again for writing back. Let me try to address the issues:
1. It is safe for vitiligo.
2. Iminoral (Cyclosporine) is a toxic drug but can be useful in vitiligo. Apples cannot be compared to oranges and so the safety of iminoral and steroids cannot be compared.
