Suggest Treatment Fr Nodules In Lungs, Inflamed Pancreas And Fatty Liver

the CTA of the chest was ordered from my cardiologist as I was having chest pain and SOB.
Again, the CTA revealed nothing with my heart and no medication was ordered.
I made an appointment on my own with a thoracic surgeon. The surgeon wanted me to come back in 6 months for a follow up. My request was to get in for follow up in 3 months at the most.
A CTA of the abdomen for dehydration also had incidental findings of an inflammed pancreas and a fatty liver.
could you please reply soon as i am not sure where my next turn should be.
Lung nodules are not a serious concern
Detailed Answer:
Hi XXXX,
Thanks for writing in to us.
I have read through your query in detail.
Please find my observations below.
1. The nodules are small and not exactly a concern if it measures less than 4 mm. As per internationally followed Fleischner guidelines, the Recommendations for Follow-up and Management of Nodules Smaller than
8 mm Detected Incidentally at CT are as follows.
(i) A nodule less than 4 mm in a low risk patient (non smoker and no other risk) requires no imaging follow up
(ii) A nodule less than 4 mm in a high risk patient (smoker) to be followed up by doing another CT at 12 months and if no change then no further imaging required
2. As CTA is normal, it is unlikely that you have a coronary heart disease. However should the shortness of breath persist then you must discuss with your cardiologist to know if you require more test like echocardiography or lung function tests at a later date.
3. Fatty liver is not a concern. The inflamed pancreas is to be taken care of by medicine and diet care. This might include intravenous fluids, enzyme and pain relief pills if required. Low fat diet is to be consumed when there is inflamed pancreas and taking plenty of water and fluids. Please reduce alcohol intake.
4. Keeping the above in mind meeting the thoracic surgeon again in 6 months is better, however should you require, an emergency appointment can always be requested.
Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.
Hope your query is answered.
Do write back if you have any doubts.
Regards,
Dr.Vivek
