Thanks for your question.
Noted your concern.
In such scenario I may suggest to increase night dose of Mixtard 30 with continuation of current medicines.
But this should be done with close monitoring of blood sugar levels with consultation with treating diabetologsit.
With increase in dose of insulin there may be possibility of hypopglycemia (low blood sugar level).
Hence in such scenario I usually suggest self monitoring of blood sugar levels with Glucometer at home.
While doing this there is a need for continuous counseling, diabetic education and frequent follow up.
In Self monitoring of blood sugar level (BSL done by glucometer at different times of the day) I may suggest to do following readings at least 2 days in a week
- Fasting,
- 2 hours after breakfast,
- before lunch,
- 2 hours after lunch,
- before dinner,
- 2 hours after dinner,
- early morning sugar 3am.
- Additional reading if there are symptoms of hypoglycemia (low blood sugar level).
Depending upon this chart further changes in insulin doses or insulin type and oral medicines can be suggested.
Along with this compliance with diabetic diet and regular exercise are important.
