Brief Answer:
urgent consultation advised, strong suspicion of mood disorder

Detailed Answer:
Dear,
Welcome to HCM
Thanks for writing in.
As i could understand here, your major worry is alternating spells of extreme mood. Presence of drinking and suicidal thoughts are further adding to the complications.

There primary conditions that we must consider is of mood disorder; a bipolar or more chronic and stable one of a cyclothymia.
For establishing a clearer clinical picture of your condition I may need to know
1) how long does the episode last? Weeks, months, days?
2) how many such individual episodes of "high" and "low" occurs in a single year?
3) history of similar complaints in family?
4) any history of childhood problems?
5) how is your family support? Family members?
6) how has the condition affected your daily work, study?

Considering the presence of suicidal thoughts, it is strongly advised to consult a psychiatrist urgently.

Kindly revert back with the information.

Dr Pankaj Borade
