I have gone through your question in detail and I can understand what you are going through.
I agree with you that your diagnosis is not something like a clear cut " text book" diagnosis. But i would like to still go in favour of bipolar mood disorder. The reasons I suppose so is.
1. You did have a long standing depressive episode mostly resistant to many different antideprssants. This is a n important component of a bipolar mood disorder.
2. You had a history complicated with substance abuse- both alcocol and vicodein.
3. The psychotic episodes that you had were very brief and responded easilt to abilify.
4. Bipolar disorders need not essentially have a manic episode. Infact even if there is an irritable hypomaic episode, even then it can be bipolar 2 episode.
5. You are currently doing good on effexor, lamictal, abilify and stimulants. Now a patient of psyhosis generally doesnt require and antkdepressant and mood stabilizer for this long. In conrst it is essential for a patient of bipolar mood disorder.
Now that you are doing well, you know what is working for you. What you need to do is keep taking medicines regularly.

Rapid cycling and mixed states are all terms used for different aspects bipolar mood disorder. Rapid cycling is stated when there are 4 or more episodes in a month. Mixed state is said to be present if during 24 hours patient has a mania as well as a depression phase.
Schizoaffective disorder will have all the symptoms of bipolar mood disorder and episodic course along with symptoms of schizophrenia with disorganized thoughts and behaviour. For you it means that bipolar mood disorder is easier to treat and has a better prognosis with interepisode complete recovery. In schizoaffective disorder there is no interepisode complete recovery and generally a bad prognosis as compared to the former. The treatment is only slightly different.
