Hello!
Welcome and thank you for asking on HCM!
Regarding your concern, I would explain that family history is an important risk factor for coronary artery disease, but there is not a specified percentage of this risk.
From the other hand there are also a lot of other risk factors which contribute in the occurrence of this disorder (male sex, age, diabetes, hypertension, dyslipidemia, lifestyle, obesity, smoking, etc.).
Based on your family history, I would recommend checking your blood lipid profile for possible hereditary dyslipidemia, which are known to cause coronary artery disease in early age in members of the same family.
As atherosclerosis (the pathological process underlying coronary artery disease), begins in early age in predisposed persons, I would recommend performing periodically scheduled follow ups every 6-12months by your attending physician:
- a resting ECG and cardiac ultrasound
- some blood lab tests (complete blood count, fasting glucose, blood lipid profile, thyroid hormone levels, kidney and liver function tests).
If suspicions of coronary artery disease are raised other tests are needed:
- a cardiac stress test
- Myocardial perfusion imaging
- Coronary angio CT scan to calculate your risk for future acute coronary events.
Meanwhile, I would recommend you to follow a heathy lifestyle (a healthy diet like Mediterranean diet, a lot of physical activity), monitor your blood pressure and avoid smoking or alcohol.
Hope you will find this answer helpful!
Feel free to ask any other questions whenever you need!
Kind regards,
Dr. Iliri

Hello again!
Thank you for the additional information!
Your symptoms don't seem to be related to coronary artery disease.
Your HDL levels are a little low and I would recommend performing aerobics and take food reach in omega 3 fatty acids (wallnuts, fish, fish oil, etc.) to help you raise HDL levels.
I would also recommend performing thyroid hormone levels and blood electrolytes as a thyroid dysfunction or electrolytes imbalance could lead to an increased heart rate (like yours).
From the other hand anxiety could also trigger all this situation.
Another point to consider would be gastritis or gastroesophageal reflux. These disorders are common causes of chest pain in young persons. I would advise you to take some antiacids or PPI (omprazole, lansoprazole) in case of chest pain. If the pain is calmed by these drugs, a fibrogastroscopy may be needed to rule in/out these possible causes.
Nevertheless, for the moment you have nothing to worry about!
I recommend continue with a healthy lifestyle and a lot of physical activity.
Hope you will find this answer helpful!
I am always at your disposal if you have any other uncertainties!
Best wishes,
Dr. Iliri

Your response concerns me I live a very healthy life style much healthier than most I workout 4 days a week with a trainer 1 hour cardio 1 hour of weights I am in better shape than most women my age (25) and out of the blue I physically can not lift weights I can't run something is stopping me from pushing myself I know I physically can but my body is telling me to stop iv had heartburn for days at a time and antacids do not seem to help (much like my brother I am very health oriented and active) I eat very clean and there isn't much about my life style that I honestly should change And all of these feelings and symptoms started about a month before my brother had his bypass ( we had no idea any of us kids might have heart issues we found out about the surgery the day before) could this be just stress? I really fear going to the doctors in general so I'm trying to avoid a visit at all but I don't want to randomly die
Brief Answer:
You have nothing to worry about!
Detailed Answer:
Hello again!
Regarding your concern, I would like to confirm that anxiety could mimic all this clinical situation.
As your symptoms are not calmed by antiacids, it is probably just anxiety.
As I already mentioned before, there is nothing to be concerned about. I would exclude any cardiac cause related to your symptoms.
So just relax and don't worry about it!
I recommend you to follow with your actual lifestyle.
You can try taking alprazolam 0.25mg or 0.5mg to help you with anxiety during these situations.
Yoga and meditation can also be really useful.
Wishing good health,
Dr. Iliri
