Question: female age 75 ,5'7'', weight about 145 lbs. she becomes dizzie ,depanding on her sitting position it become worst,very tried, always shackey on the inside and feels that she is about to jump out of her skin and on the outside appearscalm but extiemely tired and has to lay down.

the female is 75 years old, she has one daughter that is bypoler. this has been going on for at less two months, local doctor does not know why she is dizzie ans feels so nervous in the inside. she does have some lower back probles and can noy walk very fall without haing to sit doen unless she has a cart to walk behind. what kind of tests should she have to better understand her condition. this person has always been up and going and does not know why she is so tired

Thanks for the additional information sent.

Patient has multiple risk factors for depression like: Family history of Bipolar illness (episodes of mania and depression), presence of medical co-morbidity (lower back pain) and old age.

Apart from that she has dizziness, low mood (nervous), restlessness and extreme tiredness. All these symptom complex indicate toward the presence of depression.

It is difficult to judge the cause of lower back pain as it may occur in depression and also in the presence of physical illness. In such cases physical illness needs to be rule out first.

Following steps will help in management of patient:

•     MRI spine will help to rule out local pathology in spine.

•     Liver/kidney function test and complete blood count for general wellbeing of patient

•     Inform your physician about the presence of psychological symptoms and family history of psychiatric illness to seek psychiatric opinion
