1)Since when you were having this pain in left leg,shoulder,neck and arm?
2)Was this pain persistent or episodic?If episodic,what was the frequency of episodes?What were the factors/activity which used to trigger the episodes?
3)Any other medical history like Diabetes,Thyroid illness or other disease?
4)Any history of anxiety/Depression?

Do have a history of anxiety and depression ( have been in counseling for years.
The dent behind my left ear is fairly new or just noticed it.

Thanks for being in follow-up.
With the background history of anxiety and depression,the most likely possibility of your symptoms is Psychosomatic etiology.Please start doing regular mental relaxation exercises like meditation/yoga and observe for the improvement,if any.Psychiatrist's consultation should be taken to manage anxiety and depression.Managing these psychogenic issues may itself lead to improvement of your symptoms of neck,shoulder,arm and leg pain.Dent behind the ear,is most likely, nothing significant and it may have been there before also.It is your psychogenic problems which are troubling you more.If you are so worried,please get this dent examined by a general surgeon.
However,for neck,shoulder and arm pain,I don't want to miss upon the possibility of cervical spondylosis.Though,you mentioned that MRI spine was essentially normal,I suggest you to have a trial of following medicines-
1)Pregabalin 75 mg once a day
2)Methylcobalamin,vitamin D and calcium supplementation.
