I understand your worries and they are totally genuine. I will suggest that you consider stopping smoking. Also, hiatal hernia allows for easy flow of substances from the stomach back to the stomach and this could explain why there is this sensation as well.
For now, consider stopping cigarettes. Avoid alcohol, chocolates and eating soon to bedtime. Equally avoid leaning forward soon after eating as this facilitate food moving into the esophagus and throat that can cause similar symptoms.
