- is there any discharge coming out from that hole?
- is it itchy? painful?
- any other symptom you might be having recently?
- any sexual experiment in the both holes?
That hole seems to be anal fistula. However, need to rule out pinoidal cyst if discharge.

No discharge just some bleeding when I wipe it with the toilet paper , no other symptoms and no sexual things in that area
