I understand your concern.

I went through the photo you provided and I need to have a few more info regarding your actual health problems:
- is there any discharge coming out from that hole?
- is it itchy? painful?
- any other symptom you might be having recently?
- any sexual experiment in the both holes?

That hole seems to be anal fistula. However, need to rule out pinoidal cyst if discharge.

Hi,
No discharge just some bleeding when I wipe it with the toilet paper , no other symptoms and no sexual things in that area

No that's not serious. However, I should ask to get an appointment with a surgeon. To me, it's an anal fistula. However, a close physical examination is required.
