Question: I checked my brother in the hospital because I knew he was having liver failure. His stomach was huge and eyes yellow . Sure enough he has liver serorosis and they removed 10 liters of fluid off the abdomen. I cant get a hold of the doctors and I fear my brother is not telling me everything. He is bleeding from his rectum and I was wondering what that could be caused by.

Brief Answer:
impaired coagulation system

Detailed Answer:
Hello,

obviously your brother has cirrhosis. Cirrhosis causes various disorders including coagulation cascade disorders. This means that bleeding cannot stop as fast as in normal individuals and whenever bleeding occurs he may loose a lot of blood. Patients with cirrhosis also have engorged veins in the stomach and other sites of the gastrointestinal tract. These veins may bleed and cause even life-threatening blood loss.

Esophageal varices is a classical cause of bleeding in patients with cirrhosis but bleeding from the rectum in such a case would mean that the patient has lost too much blood. Bleeding from hemorrhoids is also possible. Finally bleeding from other sites of the gastrointestinal tract is also possible. Only endoscopy of the gastrointestinal tract may reveal the diagnosis with certainty.

Clinical examination may reveal clues for the diagnosis.
