There lies a possibility of repeated trivial unnoticed trauma to one's testes during
masturbation and also during the sexual activity. This could give rise to inflammation with super added infection of the testis (called
epididymo-orchitis) causing pain in the affected testis, and the pain being referred to the abdomen. This happens because testes derive their blood and
nerve supply from their original location in the abdomen.
The
chills associated point to
bacteremia in the blood due to the testicular infection.
If I were your doctor, I would advise you for a urine routine test along with a culture and sensitivity test as well, a routine CBC, and a serum PSA as a base line. Depending on the reports of the test, I would start a broad spectrum antibiotic, an anti inflammatory, and a
proton pump inhibitor drug for a short while around 5-7 days along with a good testicular support and with a advice not to stand erect for long periods of time. If not relieved, I would not hesitate to advise for a Doppler
ultrasound of both testes of yours. Further management would depend on a repeat physical examination and the report of the ultrasound.
