What Causes Abdominal Pain, Constipation And Nausea?
Question: Hello,
My sister had abdominal pain and she had colonoscopy in XXXXXXX I am going to upload the images and I would appreciate if you could let me know what is going on in her intestines.
Symptoms: When she is upset or gets tired,
* Abdominal pain
* Cramp
* Constipation
* Inflation [perhaps in stomach]
* Neusea
Medications that she has taken:
* For 5 weeks, she used 6 mg. cortisone tablets that target the intestines only. [I think it was originally 9 mg, but they reduced it to 6 mg perhaps due to side effects. Also, she was supposed to use it for 1 more week, but she could not endure the abdominal pain.]
* She used 40 mg of Pantoprazol for her stomach for 4 weeks.

Case history: Recurrent abdominal pressure sensation, nausea, constipation tendency, pain at perianal presses, blood admixture. Duodenitis, terminal ileitis (non-specific), hyperplastic rectum polyp. Control, currently no drug therapy, not taking NSAIDs. Gastroscopy, colonoscopy + proctoscopy – Anal fissure?
Esophago-Gastro-Duodenoscopy from XXXXXXX 10, 2015:
Premedication: 5 mg Dormicum, 70 mg Propofol
Findings: Esophagus: Tubular esophagus inconspicuous, Z-line with individual fibrin covered erosions, no relevant hernia. Stomach with inconspicuous mucosa conditions. Duodenum inconspicuous. Papilla inconspicuous. Drain clear bile.
Quick test for Helicobacter pylori from XXXXXXX 10, 2015:
Antrum: Negative, Corpus: Negative
Assessment: Reflux esophagitis Stadium A. Otherwise, inconspicuous mucosa conditions.

I have gone through all the images and reports you have provided. Excpt in series 2image1 there is a pedunculated polyp which on biopsy comes out to be a hyperplastic polyp which is a benign condition for which there is nothing to worry about. And in upper GI endoscopy there is few erosions in duodenum (Duodenitis).I hope there is no alarming features like loss of weight or apetite or blood loss in stool. Then it appears that your sister may be suffering from IBS (Irritable bowel syndrome). Few things to know about IBS:
1. Is a chronic disease which has relapsing and remitting course
2. It will never harm even if it will persist for whole life
3. Stress, life events, and anxiety leads to aggravation of symptoms as in your sis
4. Intestine becomes hypersensitive so even discomfort is perceived as pain
5. Increase gas production leading to abdominal fullness and bloating
6. No alarming features like weight loss, gastrointestinal bleeding, or fever
Now the solution:
1. Ask her to take high fibre diet, may add psyllium fibres to ease constipation
2. For pain take antispasmodic agent like clidinium
3. For nausea and duodenitis just add a proton pump inhibitor like pantoprazole as she is taking along with a prokinetic agent like a domperidone
4. For bloating a. see if symptoms becomes worse after eating any milk products, beans, peas if yes then ask her to avoid them
b. take capsules of simethicone which I found very useful in my patients complaning of bloating
