The presence of bilateral ovarian cysts is most likely seen in cases of polycystic ovarian syndrome cases where the woman presents with delayed and irregular menses, excessive hair growth or hyperandrogenic features and oligomenorrhoea.
Another possible cause of accompanied bilateral pelvic pain is pelvic inflammatory disease which can be treated with antibiotics in 14 days as per cdc guidelines . However it is important to take a high vaginal swab for culture to isolate any causative organism for PID , so that the treatment can be directed accordingly.
Please get an ultrasound pelvis and a high vaginal swab done first.
