Hi and thanks for the query.

Pain felt in the back secondary to GI causes are usually due to -

1) A penetrating gastric or duodenal ulcer.
2) lower end common bile duct lesions like a stone.
3) Pancreatitis.
4) rarely a bad reflux disease.

A normal OGD usually rules out an ulcer or reflux disease.

The other two can be looked at by an ultrasound or a CAT scan.

Hope I have answered your query. Please do write back if you have additional concerns.

Wishing you good health.

Regards.
