What Causes Bloating, Abdominal Distension And Acid Reflux After Eating?

Question: Every time I eat something I get really bad bloating, distended stomach, acid reflux, even if it's considered part of the bland food diet. this has been happening for about 4 days along with nausea and vomiting, low grade fever, headaches, chills, stomach pain, dark watery (literally pure liquid) stool. I do not have a gallbladder. It started off with an entire day with absolutely awful acid reflux, and it just got worse from there. Any solutions as to what it could be? Or something I can do to help it? I have a history of acute Pancreatitis and chronic(?) gastritis.

I have gone very carefully through your query. Your problem seems to be due to gastritis or gastroesophageal reflux. Your history of pancreatitis complicates the situation.

Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets hampered, food along with the digestive juices would stagnate, stomach acid will reflux into the food pipe and on the other hand the bowel habits would not be proper. Degradation of undigested food in the intestines by the intestinal bacteria would lead to the excess gas formation and bloating.

In order to relieve yourself, you need to do the following:
* Avoid spicy food. Take small but frequent meals. Avoid long gaps between meals.
* Avoid alcohol intake (if it a part of your habit).
* Take Prilosec (omeprazole) once daily before breakfast.
* Take some liquid antacid, especially the ones containing simethicone.
* Take plenty of green leafy vegetables as well as other sources of dietary fibers. Take fiber supplements like isabgol/psyllium husk or senna. I would insist you to increase your dietary fiber intake.
* Regular morning walk and exercise to keep the bowel movements proper.
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.
* Stress reduction has a crucial role to play. Yoga and meditation should help.

I do take prilosec and Zantac everyday but they are just not doing the trick anymore. Is there any other acid reducer that you would recommend that is otc? What's the difference between the liquid antacids and the prilosec?

Stress has a crucial role to play.

Detailed Answer:
Prilosec and Zantac are effective acid reducers but if they are ineffective, chances are that anything else would not help. I would not recommend any other OTC substitute. Liquid antacids neutralize the acids that have been secreted whereas Prilosec or Zantac block acid production/secretion.

I would insist you to visit a gastroenterologist and get yourself evaluated for H.pylori infection. If it is found to be positive, treatment with appropriate antibiotic regimen will help in resolution of your problem.

Stress reduction is perhaps the most important factor. If I had a patient like you I would perhaps have suggested Prodep 20 mg tablet (long term anxiolytic) once daily for at least 2 months (it may take time for the action to set in). Please discuss with your doctor.
