What Causes Blood Coming Out From Mouth?

Question: Dear Sir,
My mother Name is XXXXXXX her age is 56, from 15 days she told that very small quantity of blood is coming from mouth while normal splitting and some chest pain few times. There are no instances of fever and weight loss. few tests were done and results are given under
ESR 1st hour :14
Activated Partial Thromboplastin Time(APTT): 26.7 sec , control 31.8 sec
Prothrombin Time test :13.3 Sec , Control :12.8, Index 96.2 %, INR:1.03

Impression:
1.) Small Centrilobular Nodule measuring 4.6 *4.0 mm apical basal segments of left lower lobe
2)Fibrotic Bands with Mild traction Bronchiectatic changes in anterior basal segment of left lower lobe
3) Small Pachy areas of consolidation in inferior lingular segment
4) Fibrotic bands in medial segment of right middle lobe and anterior segment of left upper lobe
5) Small Volume pretracheal and precarinal nodes
I told my mom nothing is serious, but iam too worried and the actual doctor is on leave for a week.
Please kindly say seriousness of this issue and whether any cancer is developing in lungs. Is it starting stage and whether it is curable. Please kindly guide me how can I approach.
My mother is using aurvedic medicine for about 45 days for jaundice problem she is telling that arurvedic medice has caused this. Is this problem is because of infection in lungs due to usage of aurvedic medicine.

Thanks for your query.
The centrilobular nodule occurs most commonly due to infections especially tuberculosis. Other features on CT scan are suggestive of fibrotic and bronchiectatic changes, which may be due to past tuberculosis or other infective insults in the lungs. Though in this age group the possibility of lung cancer cannot be ignored. So I recommend the following tests in this scenario -
1. Sputum for acid-fast bacilli
2. Sputum for malignant cells
3. Broncho-alveolar lavage analysis and fibre-optic bronchoscopic evaluation of tracheo-bronchial tree
4. CT guided FNAC or biopsy from that nodule.
I think all these investigations can give you a clue to the possible etiology. If both tubercular or lung cancer work-up is found to be negative then treatment will be to control hemoptysis and infections further. This problem is not caused by any ayurvedic medications. You should consult a good pulmonologist soon.

Dear Sir,
I visited pulmologist and showed reports of blood and ct scan . he said thet lung nodule occurred because of infection and no need to worry. He said 99% it is not lung cancer but also he told to go for Chest Xray after 2 months. He told that lung nodule is very small and it is because of infection. He advised some medication to be followed for 7 days and after 7 days he asked to visit hospital again. He didn't advise further tests
Medicines advised
1. Clarinova 250
2. Trenexa 500
3.sompraz
He told nothing is serious but I read in net that lung nodules may develop and might cause problem. He told to wait for 2 months. By waiting this 2 months is there posibilty of nodule growing. Pls advise whether I should go for 2nd opinion because to wait for 2 months and then to verify ,is it a good option. Please kindly tell the medications mentioned 1,2,3 is sufficient to resolve problem .Please kindly give your advise on how should I approach from here .

Thanks for writing back.
As I have said earlier and your pulmonologist also said rightly that centrilobular nodule is mostly because of infections and your pulmonologist rightly treated her with clarithromycin (clarinova). It is internationally recommended that if the nodule is small a repeat scan is to be done after 2 months in order to see whether there is increase in size or not. If the nodule remains at the same size the possibility of malignancy is almost nil. So waiting for two months is really a good and ethical option. But I will recommend you to have a repeat CT scan after two months rather than having a chest x-ray. But as per her age is concerned the possibility of malignancy though very little should be excluded. In that case you may take a second opinion and may go for a CT guided FNAC of that above said nodule if accessible through CT guided intervention. But again I am telling you that possibility of malignancy is very very minimal here. We the doctors can never say that this treatment is sufficient. But if it is really due to infections the chance of resolution by the treatment prescribed by your doctor is high. That's why two months time is essential. If you want to have confirmatory diagnosis right now you may go for CT guided FNAC and bronchoscopic procedures right now. At least these will relieve your anxiety and you will have a definite idea on your mother's disease.

Dear Sir,
Few times small quantity of blood is coming from my mother mouth while splitting few times( once in 4 days). Do blood comes because of infection. Is blood coming from mouth is a worrying sign or it is common symptom of infection.

There are many reasons of coughing out blood (hemoptysis), the common causes of which are tuberculosis, lung cancer, pneumonia, lung abscess, bronciectasis, chronic bronchitis and few heart diseases and systemic diseases. Hemoptysis can be found in infections but it is more commonly found in tuberculosis and lung cancer especially in older age group with significant smoking history. Presence of hemoptysis warrents a pulmonologist to go for a bronchoscopic evaluation of bronchial tree.
