MND is less likely.
Detailed Answer:
Hi XXXXXXX ,
Thank you for posting your query.
I have gone through the medical reports sent by you.
I agree that the doctors have suspected/made a diagnosis of motor neuron disease (MND) in your case.
However, it is less likely. This is because MND is very unusual at this young age. It typically occurs in people above 50.
Secondly, too much pain in muscles is not a feature of MND.
Thirdly, the progression is relatively slower for MND. In a typical person with MND, the disease spreads to legs and swallowing/speaking muscles in 3-5 years.
The more likely possibilities could be multifocal motor neuropathy with conduction block (MMNCB), and segmental anterior horn cell disease. Both these conditions have better outcomes than MND.
MMNCB responds to treatment with immunotherapy (cyclophosphamide, immunoglobulins, etc).
I hope my reply has helped you.
I would be pleased to answer, if you have any follow up queries or if you require any further information.
Best wishes,

You need to have positive outlook.
Detailed Answer:
Thank you for getting back.
I can understand your frustration with lack of confirmed diagnosis and lack of any definite treatment so far.
Motor neurone disease is not a uniform disease, and the severity and progression may differ in different individuals.
As of now, I would expect you to continue with your current neurologist's advice.
In addition, you should have a positive outlook and do regular exercises.

Continue physiotherapy and riluzole.
Detailed Answer:
Thank you for getting back.
You should continue physiotherapy and exercises.
Riluzole is a good drug to stop/delay the progression of MND.
If you still progress, you can take a course of immunotherapy (with medications such as cyclophosphamide or IVIG, etc).
