Shortness of breath, head and neck pain on lying down, hot flashes and dizziness at age of 40 and history of smoking needs a little work up to find out the cause. For example , shortness of breath needs lungs and heart functions assessment , with history of smoking which is a risk factor for both pulmonary and cardiac causes that can cause dyspnea (shortness of breath) needs assessment and evaluation. Baseline tests like ekg, echocardiography, lung functions tests like Peak expiratory flow rate, PFTs with reversibility etc need to be done. Quitting smoking now would be a good idea and this itself can help.
Dizziness may be a separate entity and may just need antivert for symptomatic relief. It could be assocuated with dyspnea as well if it is due to heart , only work up will tell. Lethargy and hot flashes on males should be assessed for hormonal causes like hyperthyroidism, blood cell counts as smoking can cause raised red blood cells polycythemia and that can cause these hot flashing and lethargy.
