I am Dr.Ajay Panwar,a neurologist,here to answer your query.
Burning pain in feet and legs;fingers and hands-is very classical,rather hallmark of length dependent peripheral neuropathy. Common causes are-
1)Diabetes
Please get a blood sugar(fasting and post prandial) levels done.
2)Vitamin B12 deficiency-
If low vitamin B12 levels are detected along with increased serum homocysteine levels-then it confirms vitamin B12 deficiency(This probably is the case with you if you have elevated homocysteine levels).
Some other common causes can be drugs induced(if you have some medication history except gabapentin,please tell here),smoking or alcohol induced.
Nerve conduction studies can be done to establish the diagnosis of peripheral neuropathy.
Meanwhile you can continue with Gabapentin.If symptoms not controlled-amitriptyline or duloxetine can be added(UNDER YOUR PHYSICIAN'S SUPERVISION).Methylcobalamin will also be of help.
