PROCEDURE(S): MR BRAIN WO CONTRAST 70551
ACCESSION(S): 0000
DATE OF EXAM: 9/18/2015 9:01 AM
REFERRING PHYSICIAN: SUBHASHIE W. SARATHKUMARA
EXAM: MRI of the brain without IV contrast.
TECHNIQUE: Multiplanar, multisequence MRI of the brain was performed
without the administration of intravenous gadolinium contrast.
CONTRAST: None.
HISTORY: Essential and other specified forms of tremor Memory loss
and has a Hx of afib. ? multiple stroke
COMPARISONS: None.
FINDINGS:
Cerebral parenchyma: There is diffuse cerebral parenchymal volume
loss, slightly advanced for age. No intracranial mass lesion. No
restricted diffusion or gradient susceptibility to suggest acute
ischemia or hemorrhage, respectively.
Ventricular system: No hydrocephalus or midline shift.
Extra axial spaces: No epidural, subdural, or subarachnoid
hemorrhage. No basal cistern effacement.
Vascular system: No loss of the major vascular flow voids.
Visualized paranasal sinuses: Small mucosal retention cysts are seen
within the right maxillary sinus. The other visualized paranasal
sinuses are clear.
Visualized orbits: No ocular or retrobulbar pathology.
Calvarium and skull base: No osseous destruction.
Pituitary and Pineal Glands: No mass lesion.
Soft tissues: No significant abnormality.
IMPRESSION:
No acute intracranial pathology.

There are no ominous signs. There is just a 'shrinkage' of the brain which is more than expected for your age (all brains shrink with time). This finding is probably the cause of the memory loss problem. There are also some small cysts on the sinuses which is probably the result of inflammation in the past but should pose no problem to you.
