pain is spasmodic, it is like something is pinning.

in previous health check up ( 2 yrs back) dignosed with fatty lever.

Well, fatty liver does not cause pain usually.
Constipation with spasmodic/pinning pain could be due to colitis as the most probable cause and brought about by the hard stools that are unable to be pushed by the normal bowel movements.
Changes means is there any change in bowel habits like too much constipation or loose stools or change in frequency, consistency, smell and color.
If the constipation is present I would advise you the following:
Mild laxative like cremaffin plus to be taken at bedtime as halt tsf - tea-spoonful and increase as required to keep the stool soft and passing out to be smooth, without any pain.
Get an X-ray of the abdomen to see if any fecolith impaction in large bowel, particularly in the transverse colon area in upper abdomen.
Ultrasonography to be repeated to see if the liver is same or progressed any further, to see for the gall bladder and any other reason of your symptoms.
Also take a PPI like Omeprazole to tackle increased acid output and to heal the stomach, if present and can be confirmed by an upper GI Endoscopy if required at a later date.
