Question: Hi.. One of my relatives is coughing up dark fluid (blood or mucus - don't know). Sonography indicates all reports are okay.. But Alkaline phosphatase is 160 and coarse liver echotexture.. Otherwise the person, normally moving around.. no problem at all.. only coughing up dark fluid, when done so from deep within...

Hello! Thanks for putting your query in HCM. I am a Gastroenterologist.

Coughing of blood means there is problem in chest or is due to coagulopathy. So Chest x ray is needed to see where and what is the lesion. Also some blood tests like complete haemogram and prothrombin time are required to see whether there is coagulopathy or not

Sonography showing coarse liver echotexture may indicate either cirrhosis or fatty liver. I want to know what about the surface and margins of the liver?
What is Portal vein diameter?. Since in cirrhosis, surface and border are irregular and portal vein diameter is increased. And what about other liver function tests like Serum albumin, SGOT and SGPT

And if it is only fatty liver. Then you have to understand that Fatty liver has many cause like Hepatitis B, C, Metabolic disorders, NAFLD, alcohol intake etc. For which evaluation is required.

Meanwhile there are few medicines which I found very useful in my patients are ursodeoxycholic acid and vitamin E. Do regular exercise and reduce your weight if you are overweight, avoid alcohol if you drink.

So I will suggest you to visit a doctor for evaluation and management
