What Causes Diarrhea Upon Waking Up?

Question: Hi ! I have had diarrhea and loose stool every day for about 1 year and a half. i think its because i have had repeated and execisse antibiotictreatment for acne (tetracycline). My last course finished a year ago, and ever seens, its has been persistent and worse. No abdominal pain, bloating, and no acute diarrheas after meals. its only in the morning right after i get out of bed. Befor i went to the toilett in the mornings to, but not that early (i usaualy dident needed to go before brekfast), now i go 3-4 times before brekfast. now i think i got hemorrhoids. i get some blood on the paper when i dry, and a lot of mucus (sometimes it leaks). i can feel it "pushing" out from the rectum, and i get itchy, and like "anal-sweat", and sometimes pains thru the day, like a pinch. i have taken pictures, hope you can help me to take a look. Hi my dear,

I have gone through your medical history carefully and understood your concern.

Having diarrhea upon waking up in the morning with no pain , no bloating or diarrhea after meal is suggestive of irritable bowel syndrome (IBS).

However, colonoscopy, abdominal ultrasound and stool examination are recommended to exclude inflammatory disease of bowels.

What I have noted in the picture you sent in is external hemorrhoids. Avoiding constipation can ease problems with hemorrhoids.

Furthermore, apply prep H to help healing the already set hemorrhoids and repair the local mucosa.

Increase water intake, take fiber supplements, go to toilet each time you get the feeling, do not delay. Apply prep H after having a bowel movement. Take probiotics to restore the intestinal flora.

Can it not be dysbiosis because of all my tetracycline courses? For the last 10 years, iv been having at least 8 montly long courses, and its after my last course of 6 months, my stomach really been acting up. I have never been this bad before. I always go to the toilett when i need, i never have constipation as far as i can remeber.. Iv tried diffrent sort of antidepressant medecines, loperamide, and diet chanches and nothing helps for the loose stools, witch makes me wonder about ibs? I think that it should be some relife from what iv tried? No im on probiotics, seens 2 months. Some improvment, but no succses so far. Im going to my pharmacy to ask what they have for hemorrhoids, i dont know if we have the exact same medecines. (The picture i sent was when i was "pusching", if i dont push, i dont feel them). Is it normal to have exessive mucus discharge from heorrhoids? Thanks

1. Dysbacteriosis is also suspected, that's why I recommended to run stool examination and culture to determine/rule it out. Depending on the results, further prescription drugs will be recommended.

However, you are already taking probiotics, so that would have helped so far.

2. Yes, can have mucus discharge when on hemorrhoids, especially if inflammation or co-infection is present.

Okay, thanks for the answer. What test can be taken to determine dysbacteriosis? Iv taken stool samples to rule out inflamation, and my doctor says that samples of bacterias dont give you a clear answer because "there is no 0 value" (hope you understand what i mean?)

Do you meen that the probiotics should have help now? Cant it been that it takes a long time considering iv been bad for so long?

The diatery chances iv been doing is :lactose free dairy products, cut back om fibers, no fried or fat (pizza etc), no alchohol, no coffe, no chewing gum etc with that caind of sweetnes. I have also tried to see what happens when i do the opposite (when iv been free from work, i have stuffed down lots of "bad things" for ibs -apple, avoccado, chips, tacos, sodas, pizza etc.- and it was the same).. Its not always diarrhea, but loose an "mucchy". My stomach is like "stable-unstable" i wake up around 6/7 in the morning, and stays in bed like "half asleep" and usually around 9 i go to the loo. If im going to work earlier, it usually happens when i get out of bed right away, but not acute, i can usually hold it for a while (trying to pratice to "hold" it for a while) Wich makes me verry confused and wondering about that diagnosis..

Only stool examination and culture will determine if you really have dysbacteriosis.

Probiotics will help stabilize intestinal flora and ease your current problems.

It is good that you are making some changes in your diet as they will help improve your current problems.

From the details you provided, I still think of IBS although insist to rule out inflammatory/infection of bowels.
