Question: I experience painful breathing along with muscle aches and nausea periodically Like clockwork Lasts three to four days then disappears? Have been tested for asthma and lupus My mother suffered from pleurisy

These 3 day events of painful breathing come on about every 6 weeks
I moved 6 months ago to rural Pa They began then
I had a shoulder replacement surgery in XXXXXXX DC last May 2016
I had one episode after flying from DC to London last August

Based on the history you have shared, it needs to be evaluated on chest Xray as the symptoms like plerutic chest pain or pain with breathing needs evaluation and to start with chest Xray to rule out any infective etiology, or pneumothorax or even any bony abnormality need to be ruled out. Other causes like muscular pains can arise due to use of steroid inhalers and calcium and vitamin D defficiencies alongwith B12 etc can lead to such symptoms too so with prolong use of steroid inhalers etc the replacement for these should be continued.

If you had been tested positive for lupus then other lung complications for lupus can also be ruled out on Xray chest but still if needed other causes missed on Xray should be worked on CT chest and PFTs if needed.

A baseline EKG and Echo for right heart pressures should also be done alongwith D dimers to rule out any chances of embolus in pulmonary vessels. Meanhwile analgesics like acetaminophen for pain and asthma contour should be continued.
