Some times in depression with psychotic symptoms individuals might present with similar symptoms. You are taking Trazodon and Paroxetine and these drugs will take care of sleep as well as depressive symptoms. In my opinion addition of some low dose antipsychotic like Olanzapine or Amisulpiride would help to reduce your symptoms of fear. These medicines will reduce the fear and will help in better functioning. You can discuss with your psychiatrist for these medicines.
Please don't remain distressed due to these symptoms. A lot of individuals suffer from similar symptoms but with medicines there is total improvement in most of them.

The symptoms like hearing voices which others can't hear could be occurring due to hallucinations and this should be evaluated properly. This might occur in psychosis or in severe depression with psychotic symptoms. As all these symptoms are causing loss of focus and impairment in day to day functioning so I'd advise you to consult your psychiatrist as early as possible. As mentioned in my last answer Amisulpiride or Olanzapine can help to reduce the symptoms and you will fell better. As currently you aren't taking medicines and this can worsen your symptoms. Please start taking medicines again and don't ignore your symptoms. Once the symptoms improve then you can take consultation for pain shoulder.
