What Causes Dizziness And Fatigue Post Pristiq Withdrawal?

Question: Hi,
I've been on Pristiq 100mg for several years along with Dexamphetamine 5mg 3 times daily.
My psychiatrist recently prescribed Moclobemide 300mg (then 600mg after a week) to replace the Pristiq. I tapered Pristiq and suffered extreme dizziness during the withdrawal. I've been taking the 600mg dose for four days now along with the Dexamphetamine and the dizziness is still very bad. My state of depression is at an all time low. I googled taking both Moclobemide and Pristiq together and found that not only do they contraindicate each other, but that Dexamphetamines should not be taken with Moclobemide. My doctor is on holidays for a month. What should I do?

I read yoru question and i understand your concern. Moclobemide, dexamphetamines and pristiq altogether and individually all cause this dizziness and fatigue. Overdosing and combination use like in your case do it more by increasing the potency of each other and boosting their effect , we call it pharmacodynamic synergism.

MAO inhibitors and Dexamphetamines should not be used in combination as these can lead to serious side effects like hypertension shootouts, malignant hyperthermia and serotonin syndrome like agitation tremor and tachycardia.

I would suggest taper the dose to the minimum possible level and better skip and change this MAO inhibitor as antidepressant and replace it with some safer one. Only your doctor or any other psychiatrist can do that. I would suggest another psychiatrist consult if your one is not available and let them decide what is best for you.

One thing is for sure your symptoms are due to unnecessary combination of two medications that can have serious sequelae in combined use.
